Order A Birth Certificate With A Seal In Georgia

It's a straighforward process to obtain a copy of a birth certificate in Georgia.


At birth, all babies are issued a birth certificate assuming the mother signed for one in the hospital. However, to collect the birth certificate in Georgia you must order it from the Georgia Department of Community Health. If you have lost or need an extra birth certificate the steps to get a sealed replacement copy in Georgia are the same.


Instructions


1. Access the Georgia Department of Community Health's website to obtain the application (see References). If Internet access is not available, contact the vital records department at 404-679-4702.


2. Download the pdf version of the birth certificate request application or click on the link "filling out a form online." Print the pdf version and complete the form in ink.


3. Complete the proper information. Both forms want the first and last name of the person on the birth certificate including the person's gender, the date and place of birth, the name of the person's father and/or mother and the relationship of the person requesting the birth certificate to the person named on the birth certificate.


4. List how many copies of the birth certificate are needed. Fill out the check or money order for the amount of $15.00 for the first Georgia birth certificate and $5.00 for each additional birth certificate needed (Fees are current as of May 2010.)


5. Provide the mailing address of the person who made the Georgia birth certificate request. Print or generate the form, which depends on whether the applicant downloaded the pdf version or completed the form online.


6. Mail the form with a copy of a valid photo ID to:

7. Wait six to eight weeks for the birth certificate to arrive.
