Request A Birth Certificate In New Mexico

If you need a copy of the birth certificate for someone born in New Mexico, it is a simple matter to request one from the state---as long as you are a member of the subject's immediate family. Just pay the fee and fill out the paperwork that New Mexico requires. A certificate is available to the mother, father, spouse, sibling, grandparent and the birth registrant.


Instructions


Mail Request


1. Compose a letter that states your reason for requesting the document. Make sure the letter includes the name listed on the birth certificate, birth date, mother's maiden name and father's name. Also include your relationship to the person listed on the certificate. Provide your mailing address and/or street address and sign the letter. Instead of writing a letter, you may also download a copy of the New Mexico Birth Search Application form. Fill out the form completely and sign it.


2. Enclose a certified check or money order of $10 for each copy of the certificate. Make payments out to "New Mexico Vital Records." The check must be accompanied by either the letter or the completed form.


3. Send the check and paperwork by standard mail to:

4. Go to the State Public Health Office in Santa Fe or in Albuquerque. The Santa Fe office is open Monday to Friday, 8:30 a.m. to 4 p.m. MST at 1105 South St. Francis Drive. The Albuquerque office is open Monday to Friday, 9 a.m to 4 p.m. MST at 1111 Stanford NE.


5. Fill out the application provided at the counter.


6. Present the completed form with a $10 check for each copy of the certificate.
